Required to participate in any Georgia Southern sports camp:
1. Copy of insurance card (front and back)
2. A physical examination form signed by physician within one year from start date of camp
3. Signed release, covenant not to sure and assumption of risk form

*Teams bringing 13 players or more will receive a $25 discount per player.

The Women's Soccer Camp is open to any and all entrants (limited only by number, age, grade level and/or gender).
